A. General Documents (All Applicants)

☐ Fully completed Visa Application Form (DHA-84) in black ink

☐ Two (2) identical passport photographs (35mm × 45mm, white background)

☐ Valid international passport (at least 30 days beyond return date)

☐ Copy of passport bio-data page and all previous visas

☐ Typed self-introduction letter (with address and phone number)

☐ Introduction/recommendation letter from employer (signed with company stamp)

☐ Proof of accommodation – direct hotel confirmation (not third-party websites)

☐ Yellow fever vaccination certificate (for adults) or immunisation card (for minors)

☐ Proof of financial status – six (6) months recent bank statement (original, stamped & signed)

☐ Flight booking/reservation (verifiable)

C. Submission Notes

• All applications must be typed or written in block letters
• All bank statements must be original, stamped, and signed by the bank authority
• Payment is accepted only via POS — no cash
